Downloading PDF Forms
To download PDF forms, go to the page relating to the form and following procedure depending on the type of web browser you use.
Internet Explorer 5.5 or 6.0
Place the mouse cursor over one of the links and press the the right button of the mouse for IBM computers or right side of mouse for Apple
computers.
Select "Save Target As..". An option box will appear which will allow you to
select the location on your computer where you can save (download) the form.
The directory initially displayed will depend on the settings of your computer. Remember the directory where you save the form so you can find it later.
Select the Save button and the form will be download.
You can now use Adobe Reader or Adobe Acrobat to open the PDF form you have downloaded onto your computer.
Netscape 4.0+
Place the mouse cursor over one of the links and press the the right
button of the mouse for IBM computers or right side of mouse for Apple computers.
A menu option should then be displayed.
Select "Save Link As..". An option box will appear which will allow you to select the location on your computer where you can save (download)
the form.
The directory initially displayed will depend on the settings of your computer. Remember the directory where you save the form so you can
find it later.
Select the Save button and the form will be download.
You can now use Adobe Reader or Adobe Acrobat to open the PDF form you have downloaded onto your computer.
