Child Support

Initiating Application (family law) (Located on the Family Law Courts web site)

When is this form used?

This form is used for certain child support applications. For a list of applications, see the Child Support Applications brochure.

What else is filed with this form?

1. An affidavit setting out the facts and circumstances relied on and the grounds of the application, attaching:

  • a copy of any assessment made by the Child Support Registrar relevant to the application
  • a copy of any decision made by the Child Support Registrar or Social Security Appeals Tribunal relevant to the application and statement of reasons for that decision, and
  • a copy of any orders relevant to the application.

2. A completed financial statement or affidavit of financial affairs.

When is this form used?

This form is used when seeking to appeal from a decision of:

- the Social Security Appeals Tribunal (SSAT),  or
- the Child Support Registrar (CSR) to issue a departure prohibition order.
