Affidavit
This is a link to the Federal Circuit Court general affidavit for all jurisdictions.

You may use this affidavit in support of an application to set aside a bankruptcy notice pursuant to Rule 3.02 and/or an application for extended time for compliance pursuant to Rule 3.03 of the Federal Circuit Court (Bankruptcy) Rules 2006. Please see the guide to filling in an affidavit to set aside a bankruptcy notice and/or extend time for compliance.

You may also use this affidavit to verify your creditor’s petition instead of completing the affidavit in Part 2 of Form 6: Creditor’s petition. Please see the guide to filling in an affidavit verifying a Creditor's Petition.
