This affidavit is modified for use by a person who has:
conducted a search (or caused a search to be conducted) of the National
Personal Insolvency Index pursuant to Rule 4.06(3) of the Federal
Magistrates Court (Bankruptcy) Rules 2006; and/or
searched the office of the relevant court in relation to a judgement
debt pursuant to Rule rule 4.06(5) of the Federal Magistrates Court
(Bankruptcy) Rules 2006.
