When is this form used?

A party to a proceeding must give an address for service. This form is approved for use to provide an address for the service; see Paragraph 6.01(2)(b), Subrule 9.01(4) and Subrule 9.02(4).

General information

A party may give and address for service:

- by filing a relevant document that includes an address for service, or
- by filing a notice of address for service by using this form.

An address for service must be an address in Australia; see Subrule 6.01(3).

If a party's address for service changes for any reason during a proceeding, the party must file a notice of address for service and serve the notice on each party within 7 days of the change; see Rule 6.02.
